This book looks at the diversity of arrangements for transition from paediatric to adult care and offers recommendations for how hospital services can provide appropriate care for young people with diabetes. Focusing on the experiences of young people with diabetes, the report explores their views of diabetes services, their information needs and their ideas for improving service provision. Interviews with health care professionals and dieticians provide insights into how they work with young people and what steps they take to ensure that diabetes care is accessible.
